Job Description:

Maintain regular and meaningful contact with service users in line with the goals and risks identified in the support plan and to accurately record all such contact using the appropriate Council IT systems. You will need work in close collaboration and develop positive working relationships with a range of partners both internal and external to ensure that the correct level of support is available to all service users

Job Responsibilities:

  • Co-produce SMART support plans and risk assessments suited to each individual service user’s needs, consulting and offering choice in each aspect of the service they are offered
  • Deliver person-centred support with a holistic approach that considers all of each service user’s needs and aspirations
  • Put in place interventions to support service users to maintain their tenancy and achieve the necessary goals to prevent them from being socially excluded
  • Work with service users to develop realistic milestones towards independence
  • Based on the actions and outcomes identified in the support plan agree a review period for the plan of not more than three months
  • Develop and sustain effective partnerships and work collaboratively to ensure clients can take up employment, education and training opportunities
  • Adopt a flexible approach and manage a caseload so that the level of support provided to a service user can be varied based on their current needs

Job Requirements:

  • An ability to identify problems and find solutions that are not always obvious
  • Working in a housing or support service specialising in working with people with multiple needs
  • Knowledge of housing management and housing support issues pertinent to the client group
  • Working in a fast-paced environment and managing a diverse caseload
  • Interpreting complex regulations or legislation and communicating that information to different audiences
  • A working knowledge of Housing legislation, policy and practice, and current housing issues
  • Proactive partnership work with other service areas and agencies, both internal and external, to deliver an effective service
  • Preparing reports and case notes and present reports and other information at service user groups, working groups and other meetings
